Mid Suffolk 'Transport Needs' survey

A survey is being carried out by the 'Mid Suffolk Action in Partnership'  with 'Suffolk for Mental Health Voluntary and Statutory Partnership', ' Suffolk Link', and ' Mid Suffolk Mental Health Forum' supported by Mid Suffolk District Council.

Transport for people without a car is an increasing problem in rural areas. The aim of this survey is to find out the transport needs of residents in rural Mid Suffolk and how well you are being served by existing services (public transport, voluntary car schemes or community transport services such as LINK or Optua Transport etc ).

All replies received by 30th April 2012 will be entered into a prize draw to receive a £20 supermarket voucher. All details will remain strictly confidential & will only be used for this survey.
